How to Fill the CAC Trustee Declaration Form

Completing the CAC Trustee Declaration Form (MISC 05) is straightforward with our online form filler.

Step 1: Enter the Trustee’s Personal Details

Provide the trustee’s full name, residential address, telephone number, and email address exactly as they appear on their official identification documents and as used during your name reservation process. Double-check spellings to avoid delays during your CAC application.

Step 2: Enter the Organization’s Name

Type the full name of the Incorporated Trustee organization as it appears in your CAC application. Ensure the name is consistent across all registration documents.

Step 3: Upload a Passport Photograph

Attach a recent passport-sized photograph of the trustee. Make sure the image is clear and suitable for official documentation. We recommend using a clear background image.

Step 4: Add the Trustee’s Signature

Upload or draw the trustee’s signature in the space provided. This signature will be placed automatically in the appropriate section of the generated PDF. If you are uploading the signature as image, consider removing the background also using previous Remove.bg step.

Step 5: Repeat for Additional Trustees
